INCIDENT FORM

The following form is to report an incident that violates our Community Standards Policy. We take all disclosures seriously and will respond as quickly as we are able. Our first step with any report will be to prepare an individualized plan to address the situation, and then follow up with the person who received harm to make them aware of our plan and confirm whether or not the plan is acceptable to them.

The following questions are built to help us respond appropriately to address an incident. The more information we have, the better we can respond, but please know that you are welcome to share or ignore as much as you would like.
